Notebook for the Elderly

Whenever I am working with an elderly person I recommend that they get or I provide them with a small notebook.  This is beneficial for multiple reasons.  Share these reasons with them and their caregiver as well as write these uses in the front of the notebook.
*write down questions/concerns for the SLP, doctor, nurse and/or other professionals that they interact with
*write down strategies for whatever their weakness is as explained by professionals
*keep track of how well they are using strategies (memory, swallowing, orientation, etc)
*write down important information that they need to remember like appointments, instructions for home exercises, names of people that work with them, etc.
*write down memories for themselves and posterity
*you can also record basic information about the pt is their notebook to help them stay oriented (name, age, where they are, month, etc)
Depending on the pt there may be other important things that they should be recording.  Help them know what they should record and check back each visit to see that they are using their notebook appropriately.
